SystemOne Expands Services in the Fight Against TB in Mozambique

SystemOne started the new year with a significant win in the fight against TB in Mozambique. The Global Fund award, which SystemOne received in January 2020, will strengthen and expand the national program in all 12 provinces, connecting 210 GeneXpert point of care devices and second-line devices in central labs. The expansion will also provide a data deep dive and continue to fund the dedicated support resources that enable test results to reliably flow for program managers. All devices will connect with SystemOne’s Aspect platform, an upgraded version of GxAlert, to give decision makers within the national health system a single view of TB results across the country.

Expansion of the GeneXpert Network

Beginning in April 2014 with 67 devices, the Mozambique program has grown to 185 connected GeneXpert devices with another 25 planned in the coming year. We facilitated the reporting and analysis of over 250,000 tests results in 2019 with an expected increase to 300,000 results this year. The expanded coverage serves more patients and enables programs to test and treat TB to reduce the TB burden in Mozambique which is among the world’s highest. Critical within this effort is the identification of MDR-TB patients, those with a strain of TB resistant to antibiotics commonly used to treat TB. All programs seek to identify MDR-TB patients quickly and prioritize treatment to stem the transfer of MDR-TB. With Aspect, SystemOne offers a second solution to assist in this effort - connecting 2nd line LPA or MGIT devices in three central laboratories to a single integrated platform where health workers can view results and track the patient to treatment, reducing loss to follow up. Additionally, the project scope includes integrations with the national EMR and DISA, the national laboratory information system (LIS).

Support and Data Deep Dive

In addition to central lab device integrations, the project will feature a renewed emphasis on support. SystemOne’s in-country support staff, Edson Castiano, works to ensure that the devices stay connected, that lab managers are trained and that the NTP management is aware of offline or faulty GeneXpert devices. The team effort has managed to maintain a 71% network uptime in the past 6 months, despite the usual obstacles in remote areas including unreliable power supply, data top ups, and modem maintenance.

The connections and support enable the regular and timely flow of results to program managers in the NTP. Given the quantity and quality of existing data, the deep dive will be conducted at the beginning of the project, generating key program, results and demographic insights for program managers to address, monitor and manage toward. This is real-time data for decision making in action.
